School Forms

We are happy to complete school forms for our patients who have seen a provider in last year. If your child has not been seen in the last 12 months, we will require the patient to be seen before the forms are completed. Our policy regarding school forms is listed below:

• Please drop school forms off at the office.
• School forms will be mailed to the patient’s home.
• No faxing, picking up or emailing.
• It can take up to 72 hours for forms to get signed and mailed.
• Please make sure we have your current address on file, as this can help speed up the process.
